Hana Azizi, MD

We are delighted to announce the appointment of Hana Azizi, MD, as Director of Rehabilitation Medicine at the Weinberg Family Cerebral Palsy Center (WFCPC). In this capacity, Dr. Azizi will oversee the physical medicine and rehabilitation program at the WFCPC and establish a new multidisciplinary clinic on the CUIMC campus.

Dr. Hana Azizi is a Physical Medicine and Rehabilitation physician, the A. David Gurewitsch Assistant Professor of Clinical Rehabilitation Medicine, and the Fellowship Director for Pediatric Rehabilitation Medicine at Columbia University Irving Medical Center. She specializes in the comprehensive care of individuals with cerebral palsy, with clinical expertise in ultrasound-guided botulinum toxin injections and alcohol nerve blocks, intrathecal baclofen pump management, and mobility enhancement through adaptive equipment and orthotic management.

In the multidisciplinary clinic, Dr, Azizi will work with WFCPC colleagues from Neurology, Orthopedics and Neurosurgery on individualized medical and surgical management of complex cases. The clinic will serve patients with cerebral palsy of all ages.

"Working alongside experts in Neurology, Orthopedics, and Neurosurgery, we provide comprehensive, coordinated care tailored to each patient’s unique needs. Medically complex patients deserve nothing less than a collaborative approach—one that simplifies their journey and improves outcomes," said Dr. Azizi.

“Dr. Azizi is loved by her patients and highly respected by her colleagues.” shared the WFCPC’s Executive Director, Jason B. Carmel, MD, Phd. “We are delighted that she will work side-by-side with our multidisciplinary team to improve shared decisions and coordinate treatment.”

In the past years, the WFCPC has expanded its multidisciplinary team of clinicians and re-envisioned how and where providers from various disciplines at the Center see patients. The Center’s aim has been to co-locate multiple specialties to ensure that patients have a seamless visit, limiting the need to travel to and from various locations for appointments, and so that the dialogue amongst providers is enhanced, leading to improved clinical decision-making.
